Indication/Uses: 

1. Primary Hypercholesterolemia

   • Usage: Fenofibrate is prescribed to lower elevated levels of total cholesterol, low-density lipoprotein cholesterol (LDL-C), and apolipoprotein B in adult patients with primary hypercholesterolemia (Fredrickson Type IIa).

2. Mixed Dyslipidemia

   • Usage: It is used for the treatment of mixed dyslipidemia (Fredrickson Types IIa and IIb). Fenofibrate helps reduce elevated LDL cholesterol, total cholesterol, triglycerides, and apolipoprotein B, and increases high-density lipoprotein cholesterol (HDL-C).

3. Severe Hypertriglyceridemia

   • Usage: Fenofibrate is indicated for patients with severe hypertriglyceridemia (Fredrickson Types IV and V) to reduce elevated serum triglycerides. This can help prevent pancreatitis, a potential complication of severely elevated triglyceride levels.

Mechanism of Action

Fenofibrate works by activating peroxisome proliferator-activated receptor alpha (PPARα). Activation of PPARα increases the oxidation of fatty acids and lipolysis, which results in:

• Decreased triglyceride levels.
• Reduced production of very low-density lipoprotein (VLDL).
• Increased production of HDL cholesterol.
