4g/500mg Powder for Injection (IM/IV) Antibacterial

Here’s a comprehensive overview of Piperacillin + Tazobactam (Tazobet) 4g/500mg Powder for Injection (IM/IV) Antibacterial Vial 1's:

Product Description:

Piperacillin + Tazobactam (Tazobet) is a combination antibiotic product that contains Piperacillin (4g) and Tazobactam (500mg) in powder form for reconstitution, intended for intramuscular (IM) or intravenous (IV) injection. This combination is used to treat serious infections caused by bacteria. Piperacillin is a broad-spectrum penicillin antibiotic that works by inhibiting bacterial cell wall synthesis, while Tazobactam is a β-lactamase inhibitor that prevents bacterial enzymes from breaking down Piperacillin. This combination extends the spectrum of Piperacillin, making it effective against a wide range of bacteria, including those that produce β-lactamase, a common resistance mechanism.

Indications:

Piperacillin + Tazobactam (Tazobet) is indicated for the treatment of a variety of bacterial infections, including:

  • Respiratory tract infections, such as pneumonia and bronchitis.

  • Urinary tract infections (UTIs), including complicated UTIs.

  • Intra-abdominal infections, such as peritonitis and intra-abdominal abscesses.

  • Skin and soft tissue infections (e.g., cellulitis, wound infections).

  • Gynecological infections, such as pelvic inflammatory disease.

  • Sepsis or bacteremia.

  • Meningitis (in some cases, when specifically recommended by the healthcare provider).

  • Bone and joint infections, such as osteomyelitis.

This combination is particularly effective against Gram-positive, Gram-negative, and anaerobic bacteria, including Pseudomonas aeruginosa and Enterobacteriaceae.

Dosage:

The dosage of Piperacillin + Tazobactam (Tazobet) will vary depending on the severity of the infection, the patient’s clinical condition, and their renal function.

  • For Adults (General Dosage):

    • Typical Dose: 4g/500mg (one vial) every 6-8 hours by IV or IM injection.

    • Duration of Treatment: The treatment course usually lasts 7-14 days depending on the type and severity of the infection.

  • For Severe Infections (such as sepsis or pneumonia):

    • Higher doses may be given, typically 4g/500mg every 6 hours or as directed by a healthcare professional.

  • Renal Impairment:

    • Dosing adjustments may be needed for patients with renal dysfunction. For patients with creatinine clearance <40 mL/min, the dose may need to be reduced to prevent toxicity.

  • Pediatric Dose:

    • The dosing for children depends on their weight, age, and the infection being treated. Pediatric dosing is typically based on 80-100mg/kg/day of Piperacillin in divided doses every 6-8 hours.

Note: Always follow the specific dosing instructions provided by a healthcare provider or as indicated by the prescribing information.

Side Effects:

While Piperacillin + Tazobactam (Tazobet) is generally well tolerated, it may cause side effects. Common and serious side effects include:

Common Side Effects:

  • Gastrointestinal symptoms:

    • Diarrhea

    • Nausea

    • Vomiting

    • Abdominal discomfort

  • Rash or mild allergic reactions such as itching or hives.

  • Fever and inflammation at the injection site.

Serious Side Effects:

  • Allergic reactions: Severe reactions, such as anaphylaxis (difficulty breathing, swelling of the face or throat), require immediate medical attention.

  • Clostridium difficile-associated diarrhea (C. difficile): Antibiotic treatment may disrupt the balance of normal intestinal flora and lead to C. difficile infection, resulting in severe diarrhea.

  • Liver damage: Elevated liver enzymes (AST, ALT) may indicate liver injury. In rare cases, jaundice and hepatitis may occur.

  • Renal issues: Acute kidney injury or impaired kidney function, particularly in patients with pre-existing kidney problems or those receiving other nephrotoxic medications.

  • Hematological reactions: Blood disorders such as thrombocytopenia (low platelet count), leukopenia (low white blood cell count), and anemia have been reported.

  • Severe skin reactions: Including Stevens-Johnson syndrome (a life-threatening skin condition) or toxic epidermal necrolysis (TEN), though rare.

Rare but serious side effects:

  • Seizures, particularly in patients with renal failure or those on high doses.

If any serious side effects occur, treatment should be discontinued and medical attention should be sought immediately.

Contraindications:

Piperacillin + Tazobactam (Tazobet) should not be used in the following situations:

  • Hypersensitivity: Known allergy to Piperacillin, Tazobactam, or any component of the formulation (including other penicillin or β-lactam antibiotics).

  • History of severe allergic reactions (e.g., anaphylaxis) to penicillins or cephalosporins.

  • Severe renal impairment without dose adjustment: If kidney function is severely compromised, high doses of the medication should be avoided without careful dose adjustment.

  • Mononucleosis: Piperacillin is contraindicated in patients with mononucleosis as they are more likely to experience rashes when treated with this antibiotic.

  • Pregnancy: Category B (FDA): Animal studies have not shown harm, but there are no well-controlled studies in humans. Tazobactam and Piperacillin should be used during pregnancy only when absolutely necessary and prescribed by a healthcare provider.

  • Breastfeeding: Both Piperacillin and Tazobactam pass into breast milk in small amounts. While generally considered safe, consult a healthcare provider before using it during breastfeeding.

Precautions:

  • Renal impairment: Close monitoring of renal function is advised for patients with renal insufficiency. Dose adjustments are necessary in patients with reduced kidney function.

  • Electrolyte imbalance: Be cautious in patients with electrolyte imbalances, as the formulation may require adjustments in fluid and electrolyte balance.

  • Superinfection: As with all antibiotics, prolonged use of Piperacillin + Tazobactam can lead to superinfections (e.g., fungal infections or antibiotic-resistant bacterial infections).

  • Monitoring: Regular monitoring of liver function, kidney function, and blood counts is recommended during treatment to detect potential toxicities early.

  • Drug interactions: Piperacillin + Tazobactam may interact with other antibiotics, blood thinners (e.g., warfarin), and drugs that affect kidney function (e.g., NSAIDs). Always inform the doctor about all other medications being taken.

Note: Piperacillin + Tazobactam (Tazobet) should only be used under the supervision of a healthcare provider experienced in the use of intravenous or intramuscular antibiotics. Always follow the prescribed dosage and course to prevent resistance and ensure the effectiveness of the treatment.
