Indication/Uses: 

1. Allergic Reactions

   • Usage: Promethazine is used to treat allergic reactions including urticaria, angioedema, and allergic rhinitis. It helps relieve symptoms such as itching, runny nose, and sneezing by blocking histamine receptors.

2. Nausea and Vomiting

   • Usage: Effective in the prevention and control of nausea and vomiting associated with surgery, anesthesia, and certain medical conditions. It acts on the brain to reduce the sensation of nausea.

3. Sedation

   • Usage: Utilized as a sedative or pre-anesthetic agent. It can calm patients and induce sleep before surgical procedures.

4. Motion Sickness

   • Usage: Administered to prevent and treat motion sickness. Promethazine reduces symptoms such as dizziness, nausea, and vomiting related to motion sickness.

5. Pain Relief Adjunct

Mechanism of Action

Promethazine works primarily as a histamine H1 receptor antagonist, which blocks the action of histamine, a substance in the body that causes allergic symptoms. Its antiemetic effects are due to its action on the central nervous system, where it blocks the chemoreceptor trigger zone (CTZ) in the brain, reducing the sensation of nausea. Additionally, it has anticholinergic properties that contribute to its sedative effects by reducing acetylcholine activity in the central nervous system.

Dosage and Administration

• Adult Dosage:

  • For Allergic Reactions: 25mg injected intramuscularly or intravenously, repeated every 4 to 6 hours as needed.
  • For Sedation: 25-50mg administered intramuscularly or intravenously.
  • For Nausea/Vomiting: 12.5-25mg every 4 to 6 hours, as needed.

• Pediatric Dosage:

  • Dosage should be adjusted according to the child's age and weight, generally 0.5mg/kg/dose given intramuscularly or intravenously.
