Tobramycin (Cinarex) 3mg/ml (0.3% w/v) Ophthalmic Solution (Antibacterial Eye Drop) 5ml

Product Description:

Cinarex is an ophthalmic solution containing Tobramycin, an aminoglycoside antibiotic with potent bactericidal activity against a broad spectrum of Gram-positive and Gram-negative bacteria. It is formulated as a 0.3% w/v solution for topical use in the eyes. Each milliliter of the solution contains 3 mg of Tobramycin.

  • Indication:

    • Cinarex is used in the treatment of bacterial eye infections, including conjunctivitis, keratitis, and blepharitis, caused by susceptible organisms. The product is formulated for easy application in the eye, providing local relief from infection.

  • Presentation:

    • Available in a 5ml dropper bottle.

Indications:

Cinarex (Tobramycin 3mg/ml Ophthalmic Solution) is indicated for the treatment of bacterial infections in the eye caused by susceptible microorganisms, such as:

  1. Bacterial Conjunctivitis – Inflammation of the conjunctiva caused by bacteria.

  2. Bacterial Keratitis – Infection of the cornea.

  3. Blepharitis – Inflammation of the eyelids often caused by bacterial infection.

  4. Other Bacterial Eye Infections – Including post-operative infections and infections following eye trauma.

Cinarex is also used as a prophylactic treatment to prevent bacterial eye infections in high-risk situations (e.g., after eye surgery or trauma).

Dosage and Administration:

  • General Dosage for Adults and Children:

    • For bacterial eye infections:

      • Instill 1-2 drops into the affected eye(s) every 4 hours during the day.

      • In severe infections, the frequency of application may be increased to every 1-2 hours initially.

      • Once the infection begins to resolve, reduce the frequency of administration gradually.

  • For Prophylaxis (post-surgical or post-trauma):

    • Instill 1-2 drops into the affected eye(s) 2-3 times a day for 3-5 days or as directed by a healthcare provider.

  • For Children and Infants:

    • The same dosages as for adults are typically used, but should be administered under the guidance of a healthcare provider.

  • Instructions for Use:

    • Wash hands before using the eye drops.

    • Tilt the head back, pull down the lower eyelid, and apply the drops to the affected eye.

    • Avoid touching the dropper tip to the eye, eyelid, or surrounding areas to prevent contamination.

Side Effects:

While Cinarex (Tobramycin) ophthalmic solution is generally well tolerated, side effects can occur. Common side effects include:

  1. Local Reactions (Eye-related):

    • Eye irritation

    • Stinging or burning sensation after application

    • Redness or discomfort in the eye

    • Eye itching

    • Dry eyes

    • Blurred vision temporarily

  2. Hypersensitivity Reactions:

    • Allergic reactions such as rash, itching, or swelling around the eyes, face, or mouth.

    • Anaphylaxis (rare, but a serious allergic reaction that can cause difficulty breathing and swelling).

  3. Systemic Effects (Rare):

    • Ototoxicity (hearing loss) and nephrotoxicity (kidney damage) are rare with topical ophthalmic use, but could occur with prolonged use or misuse of the medication.

    • Superinfection: Prolonged use of antibiotics like Tobramycin can lead to the overgrowth of resistant organisms or fungi, which can result in secondary infections.

  4. Corneal toxicity (rare): Long-term or excessive use may potentially cause damage to the cornea.

If any of these side effects are severe or persistent, seek medical attention immediately.

Contraindications:

Cinarex (Tobramycin Ophthalmic Solution) should not be used in the following conditions:

  1. Hypersensitivity to Tobramycin or Other Aminoglycosides:

    • Cinarex is contraindicated in individuals who have a known allergy or hypersensitivity to Tobramycin, Aminoglycosides, or any other component of the formulation.

  2. Eye Infections Caused by Non-Susceptible Organisms:

    • It should not be used in conditions caused by non-bacterial pathogens such as viral or fungal eye infections.

  3. Use in Infants with Premature Eye Conditions:

    • Caution should be taken when using the drug in infants with premature eye conditions or underdeveloped eye structures.

  4. Use During Pregnancy and Lactation:

    • Tobramycin should be used with caution during pregnancy. While topical ophthalmic use is unlikely to cause systemic exposure, the drug should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus.

    • For breastfeeding mothers, it is advisable to use Tobramycin with caution, as small amounts of the drug may pass into breast milk.

Precautions:

  1. Use in Conjunction with Other Ocular Medications:

    • Avoid the use of multiple topical ophthalmic medications simultaneously unless directed by a healthcare provider. If other ophthalmic products are used, wait at least 5 minutes between applications.

  2. Renal Impairment:

    • While systemic absorption of Tobramycin is minimal with ocular use, it should be used cautiously in patients with renal impairment.

  3. Superinfection:

    • Prolonged use of antibiotics like Tobramycin may lead to the development of a superinfection caused by resistant bacteria or fungi.

  4. Contact Lenses:

    • Cinarex (Tobramycin) should not be used while wearing contact lenses, as it may cause irritation. Remove lenses before applying the drops and wait at least 15 minutes before reinserting them.

Conclusion:

Cinarex (Tobramycin 3mg/ml Ophthalmic Solution) is a potent antibacterial eye drop used to treat and prevent bacterial infections in the eye, including conjunctivitis, keratitis, and blepharitis. While it is generally well tolerated, side effects such as eye irritation, hypersensitivity reactions, and rare systemic effects like ototoxicity may occur. It is contraindicated in patients with known hypersensitivity to Tobramycin or other aminoglycosides. Proper use of the drops as directed by a healthcare professional is important to achieve optimal therapeutic results.
