Indication/Uses: 

  1. Skin and Soft Tissue Infections

    • Usage: Effective against a wide range of pathogens causing infections in the skin and soft tissues, including cellulitis, abscesses, and wound infections.
  2. Respiratory Tract Infections

    • Usage: Used for treating upper and lower respiratory tract infections, such as pneumonia, bronchitis, and sinusitis caused by susceptible organisms.
  3. Intra-Abdominal Infections

    • Usage: Commonly used in the treatment of peritonitis, cholecystitis, and other intra-abdominal infections.
  4. Gynecological Infections

    • Usage: Indicated for infections such as endometritis and pelvic inflammatory disease.
  5. Urinary Tract Infections

    • Usage: Treats complicated and uncomplicated urinary tract infections (UTIs) caused by susceptible bacteria.
  6. Bacterial Septicemia

Mechanism of Action

  • Ampicillin: A beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
  • Sulbactam: A beta-lactamase inhibitor that protects ampicillin from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending its spectrum of activity.

Dosage and Administration

  • Dosage: The typical dosage for adults varies depending on the severity of the infection. Commonly, 1.5 to 3 grams (ampicillin 1g + sulbactam 0.5g to ampicillin 2g + sulbactam 1g) is administered every 6 hours.
  • Route: Can be administered either intravenously (IV) or intramuscularly (IM).
  • Duration: Treatment duration depends on the type and severity of the infection, as well as patient response.

Precautions and Contraindications

  • Allergies: Contraindicated in patients with a known allergy to penicillins or beta-lactam antibiotics.
  • Renal Impairment: Dosage adjustment may be necessary in patients with impaired renal function.
  • Liver Function: Use with caution in patients with hepatic dysfunction.

Side Effects

Common side effects include:

  • Gastrointestinal disturbances (nausea, vomiting, diarrhea)
  • Rash and hypersensitivity reactions
  • Pain at the injection site

Serious side effects, though rare, may include:

  • Anaphylaxis and severe allergic reactions
  • Clostridium difficile-associated diarrhea
