Indication/Uses: 

1. Heart Failure

   • Usage: Digoxin is indicated for the treatment of heart failure, particularly in patients with chronic systolic heart failure who remain symptomatic despite standard therapy (such as ACE inhibitors and diuretics). It helps improve symptoms, exercise capacity, and quality of life by increasing the force of cardiac contractions.

2. Atrial Fibrillation and Atrial Flutter

   • Usage: Used to control ventricular response rate in patients with chronic atrial fibrillation and atrial flutter. It helps slow down the heart rate by reducing the conduction of electrical impulses through the atrioventricular (AV) node.

Mechanism of Action

Digoxin works by inhibiting the sodium-potassium ATPase pump, which leads to an increase in intracellular sodium. This promotes the influx of calcium into the heart muscle cells, enhancing cardiac contractility. Additionally, digoxin increases vagal (parasympathetic) activity, which slows conduction through the AV node and decreases the heart rate.

Dosage and Administration

• Loading Dose: For rapid digitalization in acute situations, a loading dose may be administered, which can be given intravenously.
• Maintenance Dose: The maintenance dose is typically individualized based on patient response, renal function, and serum digoxin levels. Intravenous doses are usually administered in a hospital setting.
• Administration: Can be administered intramuscularly or intravenously. Intravenous administration is preferred due to more consistent absorption and faster onset of action.
